We accept all insurace that covers acupuncture treatment. Such as: Aetna, Anthem BCBS, BCBS, Cigna, United Health, etc.

Questions to Ask your Insurance Provider:

  1. Does my policy cover acupuncture treatments?
    If "Yes", do I need a referral? If a referral is required, please contact your Primary Care Physician or Chiropractor to receive a prescription for acupuncture treatments. If you do not need a referral, please contact your employer's benefit manager.

  2. Is there a special claim form for acupuncture treatments?

  3. What kind of diagnosis is covered under my policy (i.e., back pain, post-operative pain, migraines, etc.)?

  4. How much is my deductible? How much is my co-payment per visit?

  5. What is the maximum coverage under my policy? How many visits am I authorized per year?

  6. If Dr. Lee is not on my Provider List, is there a change in my deductible or co-payment?

  7. Do I need to contact the insurance company before scheduling an appointment?
